The NP evaluates patient needs, develops and manages individualized Plans of Care, provides clinical oversight, and supports the delivery of high-quality hospice and palliative care services. Provide education and support to patients, families, and caregivers regarding disease progression, symptom management, medications, treatment plans, and end-of-life care.
