Guided by the Integrated Core Practice Model, you will use clinical judgment and discretion, in partnership with the team, to evaluate strengths and needs, choose interventions, coordinate services across disciplines, manage crises, link clients to resources, build behavioral skills, and design individualized care strategies. At least 2 years of professional, direct work with children and/or families using a mental health framework, in behavioral health, care coordination, intensive case management, social services, healthcare coordination, or related clinical services.
