Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols by: Monitoring medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, therapeutic duplications, and completeness of information. The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of a Bachelors degree in Pharmacy or a Pharm D degree from an accredited school of pharmacy or successful completion of the foreign pharmacy equivalency exam and licensure as a registered pharmacist or eligible for licensure in the State of Indiana.
