Perform tasks and documentation including but not limited to preparation of patient records, patient assessment, care planning, progress/communication notes, medication profile, pharmacokinetic monitoring, renal dosing, clinical case conferences, ongoing clinical assessment, discharge planning/summary, supply management, evaluation and selection/preparation/dispensing of medications and sterile products, supplies, and equipment as needed as directed. Clinical responsibilities include but are not limited to preparation of patient records, maintaining a medication profile, care planning, evaluation of drug interactions, preparing progress/communication notes, clinical monitoring, pharmacokinetic monitoring, renal dosing, laboratory and culture monitoring, determining medication administration compliance and effectiveness, and discharge planning.
