This role collaborates with physicians, nurses, care managers, case managers, ambulatory providers, and community pharmacies to ensure accurate medication reconciliation, patient education, and seamless transitions between inpatient, outpatient, and post-acute care settings. Qualifications: Doctor of Pharmacy (PharmD) degree from an ACPE-accredited college or school of pharmacy required; PGY1 Pharmacy Residency plus PGY2 Residency in Ambulatory Care, Internal Medicine, or a related specialty preferred.
