The role also supports transitions-of-care initiatives by coordinating bedside delivery of discharge medications through the Medication-to-Bed (M2B) program, working collaboratively with pharmacists, nurses, physicians, and the outpatient pharmacy to ensure timely filling, delivery, and review of medications before discharge. Consistently uses secure chat, email, and Epic handoff tools to communicate patient enrollment status, outstanding tasks, rejected medications, and required provider actions, ensuring clear, accurate, and timely transitions between team members and shifts.
