You will organize care and discharge planning, collaborate with pediatric discharge planners or CCS case managers when needed, evaluate patient response, and adjust treatment to support safe, effective outcomes. In this role, you will collect and assess patient data using evidence‑based methods, develop individualized plans of care, and implement treatments while coordinating with patients, families, and the care team.
