The successful candidate will assess patients, diagnose and manage acute and chronic conditions, develop individualized treatment plans, prescribe medications, monitor clinical progress, coordinate care with multidisciplinary teams and promote safe, effective continuity of care. Provide education and support to patients, families and facility staff regarding diagnoses, medications, treatment plans, symptom management and prevention of clinical deterioration.
