Considers the patient's social and structural determinants of health, including historical trauma, structural racism, and biological, social, psychological, and environmental factors in providing patient care and recommending treatments. Works to ensure that patients receive whole-person care by making appropriate referrals to OCHC's mental health and case management, eye, chiropractic, and dentistry teams.
