Provide psychosocial assessments of patients and families, coordinating care with primary care staff, to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidates experience, qualifications, location, training, licenses, shifts worked and compensation model.
