Provide technical assistance to partner organizations, enabling achievement of core and elective criteria for PCMH recognition across the concepts of team-based care and practice organization, knowing and managing patients, patient-centered access, care management and support, care coordination and transitions, performance measurement and quality improvement, and behavioral health integration. Regularly meet with steering committee composed of executive leadership from both Alaska hospitals, Moda Health subject matter experts, and statewide policy partners to guide PCMH implementation, resolve barriers, and ensure shared ownership of key decisions.
