The RoleConduct assessments to identify each participant's strengths and needs, tailoring support plans to their personal goalsDevelop, implement, monitor, and revise outcome-based services to ensure compliance with state and federal guidelinesFacilitate annual planning meetings and interim check-ins as necessary, preparing and summarizing monthly progress reportsProvide crisis intervention when needed, working closely with Behavioral Analysts to develop and track behavioral programsServe as the primary liaison for families, guardians, ISC, and external stakeholders, advocating for participants at medical and counseling appointmentsTrain and instruct Direct Support Persons (DSPs) in the implementation of service plans and proper documentationAssist with financial and insurance concerns as necessarySupport participants in engaging with community activities both on-campus and off-campusAttend and present cases to Behavioral Support and Human Rights committees, collaborating with Behavioral Analysts as neededFollow up on participant behavioral incidents in compliance with DHS and IDPH requirementsQualificationsBachelor's degree in social services, healthcare, psychology, or a related fieldMinimum 1 year of experience working with individuals with developmental disabilitiesCertification as a Qualified Intellectual Disabilities Professional (QIDP) or ability to complete certification within 6 months of hireKnowledge of state and federal guidelines (DHS, IDPH, HIPAA, Medicaid, etc.)Valid driver's license with a good driving recordMust achieve CPR, First Aid, and crisis intervention certification (CPI or NCI) through the Lambs Farm training programMust complete the Lambs Farm DSP program within 120 days of hireCommitment to ongoing professional development, including maintaining QIDP certificationPhysically capable of performing tasks that require bending, lifting, and walking, with the ability to lift and move up to 50 pounds to manage mobility aids and assist participantsProficiency in writing, speaking, and understanding EnglishProficiency in basic computer skills, including accurate typing, navigating websites, entering and recalling data, using word processors and spreadsheet software, managing emails, and familiarity with digital communication etiquette and techniquesCompensationThe salary for this role is $52,000 annually.
