We focus on promoting health equity through compassionate, culturally sensitive, and patient- centered care. Position Overview: The Primary Care Provider (PCP) delivers comprehensive primary and preventive healthcare services, addressing a range of acute and chronic health conditions. The PCP works as part of a multidisciplinary team to ensure the best outcomes for our patients, with a focus on health promotion, disease prevention, and patient education. This role involves managing diverse patient populations, including adults, children, and underserved communities. Key Responsibilities: • Patient Care o Conduct comprehensive health assessments and physical examinations. o Diagnose and manage a variety of acute and chronic medical conditions, including hypertension, diabetes, asthma, and infectious diseases. o Develop and implement individualized care plans, adjusting as needed based on patient progress and treatment goals. o Provide preventive care and wellness education, emphasizing chronic disease prevention, lifestyle modification, and health education. o Prescribe medications and other treatments in line with established protocols and patient needs. • Collaborative Care o Work collaboratively with a multidisciplinary team including RNs, behavioral health providers, social workers, and other allied health staff to ensure comprehensive, coordinated care. o Participate in case management and care coordination, ensuring continuity of care and optimal patient outcomes. o Refer patients to specialty services as needed, following up to track care and outcomes. • Community Engagement and Health Promotion o Support community outreach initiatives and health fairs, promoting wellness and preventive care in alignment with FQHC mission. o Participate in patient education efforts to promote self-care and health literacy, particularly among vulnerable and low-income populations. • Documentation and Compliance o Maintain accurate and timely documentation in the Electronic Health Record (EHR), meeting all regulatory and compliance standards. o Ensure adherence to FQHC policies and procedures, including those related to billing, coding, and quality assurance. o Participate in quality improvement initiatives, including performance measurement and reporting for quality metrics (e.g., HEDIS, UDS). • Professional Development o Engage in ongoing education and training to stay current with best practices in primary care, FQHC guidelines, and culturally competent care. o Attend staff meetings, training courses, and participate in committees as needed to support organizational goals and personal professional growth. Qualifications: • Education and Licensing o MD, DO with a current, unrestricted license to practice in the state of California. o Board-certified or board-eligible in Family Medicine, Internal Medicine, or related specialty. o DEA certification and BLS/ACLS certification required. • Experience o Minimum of 1 years of clinical experience in primary care or community health setting, preferably in an FQHC or other underserved environment. o Strong commitment to serving diverse and underserved populations. • Skills and Abilities o Excellent interpersonal and communication skills, with a compassionate approach to patient care. o Proficiency in the use of EHR systems (experience with Nextgen system used] preferred). o Ability to work effectively in a fast-paced, collaborative team environment.
