Provide mobile acute care, chronic disease management, psychiatric screening and diagnosis, and medication management * Conduct universal mental health and substance use screening across visits * Manage substance use disorders, including medication management and injectable buprenorphine when appropriate * Exercise independent clinical judgment to determine when patients require emergency intervention versus field-based care * Support proactive outreach and structured follow up for patients with complex medical and psychiatric needs * Coordinate hospital discharges, medication access, referrals, and continuity of care * Collaborate closely with community partners, case managers, and behavioral health programs to streamline transitions of care * Practice trauma informed, relationship centered care addressing medical, social, and psychological determinants of health _Safety is a priority. All street outreach is conducted in teams of at least two with established safety protocols and communication check ins._ **Required Qualifications:** * Nurse Practitioner with both Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner certification * Experience caring for complex, underserved, or unhoused populations strongly preferred * Comfort providing both primary care and behavioral health services in mobile or non-traditional settings * Ability to function independently while working within a collaborative team-based model * Interest in population-based outreach, integrated care, and innovative practice models **Compensation and Benefits:** * Compensation is between $154,097 and $207,701 per year * The compensation amounts listed may include productivity or quality/service incentives, call pay, extra shift incentives, as applicable to the position.
