Works within the Certified Community Behavioral Health Center (CCBHC) model and collaborates with clients, families, interdisciplinary treatment teams, medical providers, schools, hospitals, and community partners to provide comprehensive and coordinated behavioral health care. Coordinate with pediatricians, primary care providers, specialists, hospitals, emergency departments, schools, and other community providers as clinically appropriate to support continuity of care.
