The position builds relationships with residents and community partners in trusted local settings, helps individuals identify and address barriers to care, and connects participants with appropriate physical health, behavioral health, substance use, dental, pharmacy, nutrition, and social-service resources. INDIVIDUALS SERVED: Residents and community members of all ages within the HARVEST service area, with emphasis on rural populations experiencing barriers to prevention, health education, care navigation, behavioral and physical healthcare, and social supports.
