The Community Health Worker is part of Rural Health Connect South Jersey (RHCSJ), a community-based prevention program with the goal of identifying and engaging unisured and underinsured rural residents of Atlantic, Cape May, Cumberland and Salem Counties. Excellent customer-focused interpersonal skills to interact effectively with health care practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and cultural ideals .
