This role strengthens the ability of primary care providers, nurses, care coordinators, community health workers, social service professionals, direct-care aides, and health professions students to recognize early cognitive decline, connect patients and family caregivers to dementia care and community resources, and engage caregivers in care through organizations and networks outside of the Association. Establish, convene, and staff the Network’s steering committee (chaired by the Association) and advisory committee, including adoption of a Network charter, ensuring non-duplication across activities, and connecting the Network to the Department and existing statewide ADRD workgroups and initiatives.
