Gathers all relevant documentation needed to thoroughly investigate an incident, including, but not limited to,Life Plans, Staff Action Plans, Comprehensive Functional Assessments, Staff Schedules, Behavior Support Plans,ABC Sheets, 24-Hour Head Trauma Monitoring Sheets, Diet Plans, Clinic Visit Summaries, Urgent Care/Emergency Department Paperwork, Occurrence Reports, Body Check Sheets, Staff Training Documents, Law Enforcement Complaint Forms, Photographs, Physical Evidence, etc. The QA Investigator gathers and analyzes evidence, interviews involved parties, prepares clear investigative reports, and recommends corrective actions to strengthen program practices.
