The CCG provider will work closely with the CCG Medical Director and facility staff, with the primary focus being completing the skilled admissions and responding to any acute needs, maintaining strong relationships, facilitating excellent communication and building strong partnerships between PCHC and the facility, while providing excellent care are vital in this role. PCHC's Community Care and Geriatrics (CCG) service provides care for the adult and geriatric population in area nursing homes, long-term care facilities and assisted living facilities in Bangor and surrounding areas, including Belfast, Camden, and Skowhegan.
