This role works closely with referral sources, healthcare partners, patients, and families to provide education, answer clinical questions, and support a smooth transition into hospice care when appropriate. Build and maintain relationships with hospitals, physicians, discharge planners, case managers, skilled nursing facilities, social workers, and other healthcare referral sources.
