Document all encounter information accurately, completely, and in a timely manner documenting signs and symptoms observed, plan of actions, education of juvenile, expected or observed outcome, unusual circumstances, outside communications, explanation of refusal for treatment, discharge summary if juvenile is transferred to other facilities.
