p>What Youll Do: • Manage a diverse home care caseload with autonomy and flexibility • Perform comprehensive nursing assessments and develop patient-centered plans of care • Collaborate with physicians and interdisciplinary team members to coordinate high-quality care • Monitor patient progress, adjust care plans, and educate patients and families on health management • Complete OASIS assessments with accuracy • Document care accurately and timely during patient visits • Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed • Participate in case conferences and discharge planning • Follow the Nurse Practice Act, agency policies, and home health regulations.
What You Bring:
• Current NH or Maine Registered Nurse (RN) license or other compact state license (or eligibility) • Minimum 1 year of clinical nursing experience preferred (home care experience a plus) • Excellent clinical assessment, communication, and organizational skills • Ability to work independently while being part of a collaborative care team • Comfort working in a variety of home environments • Reliable transportation and valid drivers license (Mileage reimbursed!).
