Understanding that continuity of care is a major step in recovery, you assist in determining the appropriate patient program, conduct pre-admissions assessments on prospective patients, review their medical records, discuss patient needs with the interdisciplinary team and medical staff, educate the patient and family on our programs, and facilitate the most appropriate level of care.
