As an Elara Caring Clinical Team Manager, you'll contribute to our success in the following ways: Provides oversight of all patient care services and personnel including: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate).
