RN Field Case Manager: The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois.
li>Manage transitions of care episodes for patients on their panel, including timely completion of structured clinical assessments to identify post-discharge needs, medication reconciliation on behalf of the PCP, and addressal of identified needs directly or via collaboration with other team members. Role Description: In partnership with the primary care provider, (PCP), the RN, Case Manager is the lead for care management activities, drives care coordination and collaborates with interdisciplinary teams to ensure care continuity for complex patients.
