In close partnership with Nephrologists, Registered Nurses, Social Workers, and Patient Care Technicians, you will empower dialysis patients through customized Medical Nutrition Therapy (MNT) that enhances treatment outcomes, improves laboratory measures, and elevates quality of life. In this role, you will deliver comprehensive nutritional care and guidance across our outpatient dialysis facilities and home modality programs: Conduct comprehensive initial nutrition assessments and ongoing reassessments following clinical guidelines and CMS regulations.
