Assume a leadership role in the development, implementation and refinement of day center systems and coordination of schedules which involves rigorous communication and collaboration with all departments, including transportation, the clinic, and rehabilitation therapy, recreation therapy, dining services, home care, social work, maintenance and environmental services. Overview: Under the supervision of the PACE Director and under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members.
