The Stroke Nurse Navigator participates in the development, implementation, monitoring, and sustainability of stroke program initiatives, quality improvement activities, regulatory compliance efforts, and performance measures associated with maintaining Comprehensive Stroke Center certification. Through collaboration with inpatient, outpatient, rehabilitation, and community partners, the navigator works to improve transitions of care, support evidence-based stroke management, and promote positive outcomes for patients, providers, and the health system.
