Manages the care of the high-risk ventricular assist device (VAD) patient population across the continuum (acute care inpatient, within the home, physician office, VAD Clinic, and extended care facilities) in collaboration with the Medical Director, cardiac surgeons, cardiologists, and other members of the multidisciplinary team. In conjunction with physicians and program director, program development may include but is not limited to: Outpatient clinical care forms, patient and physician professional communication tools, clinical care protocols, policy and procedures VAD patient data tracking methodology (using 4D).
