Provide skilled nursing care to home health patients including Start of Care (SOC), Resumption of Care (ROC), recertifications, revisits, and OASIS discharges. Participate in on-call/triage rotation, expected one to two weeks per month (5:30PM-8:30AM on weekdays), primarily phone-based triage with rare home visits required.
