Monitors and evaluates remote ventilator/equipment, patient usage and physiologic indices via available web platforms as needed Participates in scheduled (and as needed) inpatient care coordination meetings for CAPE patients (e.g., Proactive Case Management Rounds for MSICU) to anticipation discharge needs and optimization of disposition. Serves as a liaison and respiratory educator for community based services, including but not limited to rehabilitation programs, durable medical equipment companies, home nursing agencies, school programs for support of CAPE Program enrollees and their families.
