The School Social Worker will complete all clinical documentation, conduct diagnostic and biopsychosocial assessments, coordinate care with families, community-based providers, government agencies, and others, as appropriate, and provides consultation services as needed. Additionally, the School Social Worker functions as the internal case manager and team-leader for their assigned caseload of students and actively participates in all Individualized Education Plan (IEP), treatment planning, and caregiver meetings.
