The social worker works under the supervision of the Director of Social Services or Administrator to assist each patient/resident in reaching their maximum level of social and emotional functioning by promoting, interpreting and advocating their needs to family members and facility interdisciplinary team. Those tasks include discharge planning, care planning, discharge notifications, PASRR forms, Medicaid applications, psychoactive monitoring, behavior monitoring, social and psychosocial needs of the residents.
