Serves on an interdisciplinary team to identify and support patients in need of Care Management services, performing assessments of clinical and social status, and determining medical behavior and social needs.
div>Experienced Licensed Clinical Social Worker sought to work in a busy integrated small rural facility (FQHC), located in beautiful Downeast Maine.
Provide ongoing behavioral health therapy services individual (adult, child and adolescent per license), couples, family and group counseling; as well as brief consultations for clients referred for mental health services.
Qualifications: Minimum Requirements: Six (6) years of education and/or professional experience in social services casework, social services program development/delivery Active Licensed Social Worker (LSW) issued by the Maine State Board of Social Worker Licensure. Working within the Complex Case Unit (CCU), this role coordinates interdisciplinary providers and community resources to achieve safe discharge planning and service access in the least restrictive setting.
p>The Behavioral Health Counselor is a licensed (or conditionally licensed) therapist providing outpatient psychotherapy treatment to patients including clinical assessment, diagnosis, treatment, ongoing risk assessment and crisis management. Ability to provide case formulation and develop focused, goal-oriented treatment plans related to patient's treatment readiness or developmental level and problem identification.
This position involves professional social work with long-term care residents and their families, assisting in their adjustment to the facility, and working as a team member to provide for and assure quality of life for facility residents. Ability to establish and maintain effective working relationships with other social workers, institutional and public officials, and other community contacts.

Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses on symptoms of the terminal illness. Assess the psychosocial status of patients and families/caregivers related to the patients terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
Other system services include the Central Maine Heart and Vascular Institute, a regional trauma program, LifeFlight of Maine''s southern Maine base, the Central Maine Comprehensive Cancer Center and other high-quality clinical services.
