The Community Health Supervisor provides leadership, supervision, and operational oversight for Community Health programs, The position is responsible for managing Community Health Workers and related staff, overseeing community care coordination and referral workflows, digital navigation, health literacy, and ensuring compliance with project contracts, and fostering strong community partnerships. All YNHS employees are expected to develop and maintain professional, courteous, and collaborative working relationships with patients, families, staff, volunteers, students, healthcare professionals, community partners, vendors, and representatives of government and social service agencies.
