Job Summary: The Trauma Program LCSW is an integral member of the multidisciplinary Trauma team and provides comprehensive psychosocial assessment, crisis intervention, patient and family support, resource coordination, behavioral health linkage, longitudinal follow-up, and discharge planning for injured patients across the continuum of care. -The Social Worker will screen trauma patients from admission, conduct ongoing chart reviews, identify psychosocial and behavioral health needs, coordinate interventions and referrals, and provide continuity of psychosocial care throughout the patient's hospitalization and following discharge.
