S/he is responsible for the implementation of all activities including case management, and the assessment of patients, creating client care plans, facilitating case conferences, and collaborating with Dental Care Team members and Operational Leadership to establish sustainable pathways that ensure Oral Health treatment plan compliance and completion. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.
