<ul><li>Identifies and recognizes abnormal symptoms/changes in patients’ condition and takes appropriate action and/or reports findings to physician.</li><li>Utilizes nursing process for patient care to assess, plan, intervene, and evaluate to achieve desired patient outcomes according to the RN scope of practice.</li></ul></li><li>Develop/review/revise and individualize the plan of care to address the patient's nursing care needs for optimum patient care outcomes.
<ul><li>Integrates appropriate standards of care into the patient’s plan of care based on the diagnosis and patient-specific needs</li><li>Implements interventions that are consistent with the established plan of care in a safe and appropriate manner</li><li>Assures the plan of care reflects continuing care needs and/or referrals for discharge/transfer.</li><li>Contributes to the discharge planning process by working with the interdisciplinary care team to set patient goals and assist the patient to progress towards a safe discharge plan. Provides discharge education to patients and caregivers.</li></ul></li><li>Completes care tasks for assigned patients based on nursing process and in accordance with physicians’ orders according to the RN scope of practice. These tasks may include but are not limited to: Provides supervision and coordinates and delegates patient care tasks to members within the nursing department and interdisciplinary care team as appropriate.
