Serve as the primary care coordinator, integrating multidisciplinary efforts across the Care at Home continuum: collaborate and communicate extensively with Mobile Integrated Health (MIH) on home visit findings and escalation, with Patient Experience Navigators (PENs) on patient onboarding, scripting, and experience issues, and with Tele Health Coordinators on visit scheduling, technology troubleshooting, and equipment logistics. The RN leads care coordination across a multidisciplinary healthcare team- including Mobile Integrated Health (MIH), Patient Experience Navigators (PENs), and Telehealth Coordinators - driving therapeutic interventions, aligning logistics, and ensuring a seamless, high-quality and safe patient experience across care settings.
