This is accomplished by sharing information, and making referrals to various local, state, and federal resources; through collaboration and advocacy with those resources on behalf of families/individuals; linking families to education and training opportunities and developing short and long-term person centered futures planning to identify goals, and initiate actions to make those goals a reality.

Coordinates the life cycle of monthly electronic documentation for the Individual and Family Support department including downloading, tracking, sorting, submitting to Case Managers/Service Coordinators and to our Records Management system for electronic filing.
