This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. As part of the interdisciplinary team, appropriately renders care to mental health patients, including, but not limited to, prescribing medications, to include initiation, continuation, discontinuation, and altering therapy, based upon established formulary or protocols under the Clinical Pharmacy Specialists Scope of Practice; physical assessment; monitoring medication regimens; ordering and analyzing necessary laboratory tests; adjusting medications; and assisting in the management of medical emergencies, adverse drug reactions, and acute and chronic disease states.
