Our Nurse Practitioners and Physician Assistants deliver specialized Cardiology and Pulmonology care directly to patients in Skilled Nursing, Long-Term Care, and Post-Acute Care communities, helping improve outcomes for medically complex patients while building meaningful relationships with those they serve. This position serves patients throughout the Tinley Park area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Tinley Park, Blue Island, Oak Lawn , and surrounding communities.
