The Anchor NP enhances timely access for health center patients and improves the patient experience for patients on the panels of the primary care providers in their department by: Providing direct patient care as needed or as requested to patients on the panels of the primary care providers on the team; Providing enhanced/more frequent care to high risk patients and patients with chronic diseases; Providing specialized care responsibilities for patients with certain conditions at the discretion of the Medical Director; and. The Health Center serves a diverse community and offers an array of services including primary care, urgent care, behavioral health, care management, nutrition, family planning, optometry, dentistry, laboratory, radiology, pharmacy, and medical specialty services, including HIV care, Substance Use care, Hepatitis C treatment, OB-GYN, Nephrology, Cardiology, and an STI clinic.
