For our services, we have been recognized as one of the nation's top 100 performing home health agencies by HomeCare Elite.Qualifications<ul><li>High School Diploma or GED</li><li style="line-height:normal;vertical-align:baseline">Valid Driver's License and Current Auto Insurance</li><li>Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required</li></ul> Responsibilities include but are not limited to:<ul><li style="line-height:15pt;background:#ffffff">Volunteer functions encompass those tasks, duties, and responsibilities that are critical and fundamental to the volunteer position and comprise the means of accomplishing the job's purpose and objectives. </li><li style="line-height:15pt;background:#ffffff">Assists team with patient care in accordance with the plan of care.</li><li style="line-height:15pt;background:#ffffff">Provides needed services to patients/families and as outlined in the Hospice Volunteer Handbook (e.g. companionship, shopping, respite, etc.)</li><li style="line-height:15pt;background:#ffffff">Makes a time commitment of 2-4 hours each week as schedule permits.</li><li style="line-height:15pt;background:#ffffff">Provides notification when a substitute is needed.</li><li style="line-height:15pt;background:#ffffff">Uses listening skills effectively to provide emotional support to the patient/family.</li><li style="line-height:15pt;background:#ffffff">Reports and/or records any changes in the patient's condition or environment to the Hospice Volunteer Services Coordinator, case manager, or other appropriate team member.</li><li style="line-height:15pt;background:#ffffff">Provides accurate, timely, and objective documentation of volunteer activity.</li><li style="line-height:15pt;background:#ffffff">Submits annual mandatory paper to maintain active status as a volunteer.</li><li style="line-height:15pt;background:#ffffff">Attends scheduled rehearsals and assigned bed-side or community presentations (if applicable)</li><li style="line-height:15pt;background:#ffffff">Is able to demonstrate competency requirements and attends educational in-services and workshops as schedule permits.</li><li style="line-height:15pt;background:#ffffff">Accountable for and expected to integrate VNAHSR's HEALTH Matters values into the execution of the essential functions of their volunteer work. </li></ul>

Position is Part-Time - Friday, 11 AM to 7 PM, Saturday and Sunday, 8 AM to 4 PM.General DescriptionResponsible for oversight of shifts including: site operations, staffing, staff development, consultation, supervision, documentation requirements, site beautification. Participate in implementation of individual action plans for person’s served.  Work as a member of a team to provide opportunities for individuals while displaying hope and optimism.Essential Job Functions<ul><li>Provide supervision to direct care staff at both sites. Manage call-outs and consult with Program Director, Nurse Manager, Program Coordinator and/or Acute Care Administrator on Call</li><li>Monitor and insure completion of all medication and communication forms for next day appointments.</li><li>Monitor and update all individual records as required.</li><li>Monitor and maintain compliance of clinical records, including all MSDP and DMH standards.</li><li>Maintain work environment, manage and report hazards to supervisor and/or Facilities Manager.</li><li>Report any facilities related issues to Facilities Manager.  Follow up to insure facilities issues are corrected.</li><li>Ensure programming is active and within the scope of wellness and recovery principles.</li></ul>Qualifications, Experience and Education<ul><li>Associates or Bachelor’s degree in related field or four years of experience in related field </li><li>Knowledge of community resources </li><li>Knowledge of licensing requirements and skills to maintain those standards</li><li>Ability to supervise, evaluate and motivate staff</li><li>Ability to work a flexible schedule</li><li>Ability to multi-task and deal with a wide variety of people</li><li>Driver’s License and use of personally insured vehicle</li><li>Ability to pass a CORI (Criminal Background) check and DMV (Department of Motor Vehicles) check</li><li>Ability to make independent decisions based on defined recovery goals.</li><li>Leadership skills required to supervise and delegate responsibilities to other employees, students and volunteers and oversee assignments.</li><li>Proficient computer skills in using web based e-mail and ability to data enter information into spreadsheets and word processing documents</li></ul>
