Come join our dedicated staff!St. Joseph Hospice is seeking a PRN Chaplain/Volunteer Coordinator to join our team!We invite you to become a part of one of the fastest-growing Home Health Care and Hospice agencies that spans across six states and employs over 2,400 healthcare professionals. Our vast network of healthcare providers allows us to provide optimal patient care and seamless transition across service lines.Job duties:<ul><li>Participate in IDT meetings & will assist the IDT in evaluating and assessing patients and/or family spiritual needs</li><li>Maintain proper records of visits as determined by the guidelines and regulations of hospice services</li><li>Arrange for on-call availability of Chaplain services</li><li>Work with staff, clergy and community groups to enhance their sensitivity to the spiritual concerns of patients and families</li><li>Plans, organizes, and coordinates all bereavement activities</li><li>Responsible for bereavement to patient’s family and caregivers as set forth by Medicare hospice guidelines</li><li>Ensures documentation of all bereavement activities on a clinical note to be incorporated in the clinical record</li><li>Clerical duties as maintaining organized volunteer records</li><li>Provides a comprehensive orientation and training program</li></ul>Qualifications:<ul><li>Graduate of accredited seminary or school of theology or appropriate certification in pastoral ministry</li><li>Ability to be open, flexible and ecumenical</li><li>Make patient visits at their place of residence</li><li>Comfortable in working with patients and families with life-threatening illness and death</li><li>Documented experience in care of bereaved</li><li>Ability to work well with people, communicate efficiently</li><li>Time management skills</li><li>Must have a current LA driver’s license</li><li>Hospice or healthcare setting experience preferred but not required</li></ul>Exceptional People providing Exceptional Care.All inquiries will be kept confidential.
