These services include assessments, comprehensive service plans, and linkage to resources to help clients and their families live healthy, productive lives.</p><p>Under the general direction of the Program Director, the Outreach Coordinator and Systems Manager (OCSM) will collect, analyze, monitor, and report data related to the Hope Care Management Health Homes program.
