The Aftercare Coordinator leads trauma and burn survivorship initiatives, including the Trauma Survivors Network (TSN), Phoenix Society SOAR (Survivors Offering Assistance in Recovery), Journey Forward, peer support programs, support groups, and survivor engagement activities. This role serves as the primary liaison between inpatient care, outpatient clinics, rehabilitation services, peer support networks, and community resources to ensure a seamless continuum of care throughout recovery.
