1-3, or more, years of experience in hospice or home health community liaison work, SNF or ALF admissions or community relations, discharge planning, case management, social work, or a similar referral-side healthcare role. Our referrals come from the people responsible for a patient's next step: discharge planners, case managers, DONs, physicians, assisted living administrators, and hospice and rehab staff.
