You can find success in this role if you've held the following positions: Hospice Registered Nurse, Home Health Registered Nurse, Palliative Care Nurse, Med‑Surg Registered Nurse, Oncology Nurse, Geriatric Nurse, Field Nurse, Visiting Nurse, Admissions Nurse, Patient Care Nurse, RN Care Coordinator, or Clinical Nurse in acute care, home health, or community‑based settings. As a Hospice Registered Nurse (RN), you'll work at the center of an interdisciplinary care team, building trusted relationships with patients, families, physicians, and teammates.
Complete thorough documentation of patient and/or resident care, including nursing intervention, patient and/or residents response, needs, problems, capabilities, limitations and progress towards goals in a timely manner. Conducts an individualized assessment and evaluates the total needs of the patient and/or resident, prioritizing the data collection based on the patient's immediate condition or needs within 24 hours of admission.
